Filling out the form

You have many ways to get a voter registration form. Whichever way you get it, be sure it is postmarked by the registration deadline of May 19 for this election:

  • In person: Pick up a form at most public libraries, post offices, DMV offices, social service agencies, and at your county elections office. You can fill out the form at home and then mail it in.
  • By phone: Call any of the following toll-free numbers and ask to have one mailed to you. Be sure to do this at least one week before the registration deadline. A form will be sent to you, and you fill it out and mail it in.

English: 1-800-345-8683 Vietnamese: 1-800-339-8163

Spanish: 1-800-232-8682 Japanese: 1-800-339-2865

Chinese: 1-800-339-2857 Tagalog: 1-800-339-2957

  • Online: You can fill out most of the form using the California Online Voter Registration form. Be sure to do this at least three weeks before the registration deadline. Then a printed copy will be sent to you so you can complete the form and mail it in.

When you fill out the form, be sure to include your driver’s license or other identification. And don’t forget to sign it!
